Please fill out the form, making sure you enter your name as it appears on your pocket card for con ed registration. (If not yet licensed, just enter 000)
Enter the date of the class desired as indicated on the form (2 digit day/2 digit month/4 digit year)
If a virtual Zoom class, the location is ZOOM
Upon clicking on “Register”, you will be connected to the page for submitting your payment.
